Understanding the Challenge of Difficult-to-Heal Wounds

Difficult-to-heal wounds often arise from factors such as tissue loss, comorbidities, and inadequate blood flow. These complex wounds demand specialized care that goes beyond traditional treatments. One of the most effective solutions is the use of skin substitutes—materials designed to provide coverage and support for wound healing.

What Are Skin Substitutes?

Skin substitutes are either biological or synthetic materials that temporarily or permanently cover wounds. They are designed to restore skin functionality over time, making them crucial for managing severe or chronic wounds. These substitutes replicate the properties of healthy skin, promoting regeneration and accelerating the healing process

Types of Skin Substitutes

Temporary Skin Substitutes:

Ideal for short-term coverage, these substitutes protect the wound and create an optimal healing environment. Examples include human amnion, human allograft, and synthetic options like Biobrane and TransCyte.

Permanent Skin Substitutes:

These provide long-term coverage and improve skin quality at the wound site. Notable examples are Epicel, Alloderm, and Integra, each serving different needs, from burn care to soft tissue reconstruction.

Our Wound/Fungal Panel (P035) includes testing for a variety of bacteria and fungi that can delay wound healing and pose serious health risks if untreated.

We test for the following bacterial pathogens:

Stages of Wound Healing

1. Bleeding

This initial stage occurs immediately after an injury, where the body works to stop bleeding by forming a blood clot. Platelets gather at the wound site to form a clot, preventing further blood loss.  

2. Inflammatory
The inflammatory phase involves the body’s immune response. White blood cells, including macrophages and fibroblasts, are sent to the wound site to fight infection and remove debris. A scab forms over the wound, protecting it as healing progresses.

3. Proliferative
During the proliferative stage, new tissue begins to form. Fibroblasts produce collagen, while new blood vessels develop to supply oxygen and nutrients to the tissue. The wound starts to shrink, and a protective layer of skin covers the area.

4. Remodeling
In the final remodeling phase, the wound matures as new tissue strengthens and contracts. Collagen fibers reorganize, and the wound becomes less visible as scar tissue forms. This phase can last for several months as the skin regains its integrity.
